  • Modification Request Submit this form for approval by the Board/Mgmt Co. PRIOR to any modifications to the exterior of a building or areas surrounding a building.

  • Request to Lease out Unit Submit this form for approval by the Board PRIOR to any lease being signed by a tenant.​​

  • Designated Voter Registration (DVR). Use this form to let the management company know whom from your unit will be voting at Meetings of the Association, and also receive mailings and notifications from the Association. (For units with more than one owner, or for units where there's a Power of Attorney involved).

  • Pet Registration Form. All pets must be registered with the Association (Bylaws Art VI, Sec 7).  If you have a photo of your pet to submit with the registration - even better. This will make it easier to contact owners and return any pet that got away, while avoiding Macomb County Animal Control involvement.
